Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (407) 846-3166 during regular business hours.
Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

Various insurance companies require an inter-office referral which you must bring with you if we have not received it prior to your appointment. If a co-payment is due for your visit, the co-payment is due the day of your appointment.
If you have had any testing (MRI, X-Rays, CT scan and/or an EMG) done, please bring a copy of the report along with the actual study if applicable. You will be rescheduled if you present to your appointment without the studies. You may pick up these films at the imaging center where the test was performed. Please call the facility 24 hours in advance to have your films or compact discs ready.
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

For insurance plans where we are a participating provider - all co-pays and deductibles are due at the time of service. In the event that your insurance coverage changes to a plan where we are not a participating provider, the balance is your responsibility. If you do not have insurance, we do offer, on or before the date of service, a 50% discount for patients paying cash.

We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.
We make every effort to stay on schedule, although emergencies arise.
We only renew prescriptions that we initiated. For other medications, please contact the health professional who prescribed them.
We can fill many renewal requests by phone. Please contact our office at least ten (10) business days before you expect to run out of medication. Sometimes, for medical reasons, we will need to see you before issuing the prescription.
After-Hours & Emergencies
